If you are applying for a Spanish residency visa, your application will require proof of private health insurance. This applies to the routes most people use when moving from outside the EU:

The Non-Lucrative Visa (NLV), used by most retirees and financially independent applicants
The Digital Nomad Visa, for remote workers and the self-employed
The Student Visa, for longer study stays

For a consulate to accept your policy, it has to meet specific conditions. It must provide full coverage with no co-payments, be issued by an insurer authorized to operate in Spain, and cover the period required by your residency. A standard travel or holiday policy does not qualify.
